Intake & Assessment Process

*Patient Portal link will be sent to your email directly by our front office Admin Administrator. Initial appointment link will be sent to your email by our intake team the day of your appointment

Complete Legal Packet with Consent paperwork online or in the office prior to appointment time in order to be seen (Assessment cannot be completed without it). Meet with Assessor to complete your Biopsychosocial Health Assessment (BHA), Treatment Plan & Safety Plan. Based on the assessment, the assessor will recommend initial and immediate follow-up appointments, which may include a Psychiatric Evaluation, Medication Management, Previous Therapy review, Psychological Testing and/or Substance Abuse Treatment Services. A combination of these treatments may be recommended at the time of service. After the assessment is complete, must be scheduled and completed Psychiatric Nurse Assessment prior to beginning therapy services. Members of same family may schedule concurrent appointments with different assessors/nurses within the same hour block to encourage and expedite family therapy services.

Your Initial Treatment

Make sure to attend your scheduled appointments to connect with your therapist, paraprofessional psychiatrist, psychiatric nurse practitioner and psychologist. This will ensure you remain compliant with program requirements and minimize the risk of discharge. Take the first step towards a healthier you by reviewing and actively engaging with your treatment plan.

Continue Treatment with Recommendations

Follow all recommendations and continue your treatment as advised. Be sure to schedule any necessary follow-up appointments immediately after each visit. Same-day cancellations and no-shows will be charged at your next appointment.

Please note that psychological reports take up to six weeks to process, while request for medical records may take up to 14 days to fulfill.
